When one of the Uchiha clan married a Sunaek the Sharingan was available for that family and that family passed it down generations to generations. Their crest is a half sun in the water. All of the clan divided and went into different countries because there is a Akatsuki member named Azere, Ukias brother, that killed part of the clan with a sword named Aregan.

That's because:
He was trying to save his friend Obito when Obito got crushed under rocks.
Earlier that day, he forgot to give Kakashi a 'Congratulations' gift for Kakashi becoming a Jounin.
The only thing he could offer at the time was his sharingan.
Rin, being a medical ninja, took out the sharingan and placed inside Kakashi's eye-socket.

Kakashi has a scar because his normal eye was cut by an enemy ninja.
