The item seems to be a hat that looks like Mokona, if you're familiar with the series. And yeah, you will get repeated GENERAL ERRORs. It's pissing me off.

Masamune 5.0 I finally managed to get the damn hat. I was hoping for something better but this will do. I completed the quest, but when I looked in my inventory, there was no hat. Now I have to find more of those damn small feathers!!!!

You need atleast three of each to get the hat. And the hat won't appear in your "Customize Me" inventory. You have to go to "My Stuff" and click on it and choose to equip it.
