My rational thoughts on Susanoo:

Its a jutsu and was created upon focus (through a dojutsu) and intention thus if you are unconscious or dead you wouldn't be able to sustain it. If it were a summon on the other hand then that would be different. But its a soul projected through a technique.

Notice how it fades as Itachi dies, and completely goes away when he's dead. Why does the technique begin to fade before he is dead?

Notice how the chakra like "Muscles" receded from the skeletal structure before Itachi dies. He was conscious since he had just touched sasuke's fore head and his arm didn't go limp until a few scenes later. So you know he's alive when susanoo is falling apart. Thats why I think its a loss of focus rather than death itself. Like I said its a jutsu and jutsus dispel when a ninja loses consciousness.

I might be wrong since this the technique was a last resort after a long hard battle but I mean he didn't hold the technique up long either.
