Since All-Ages Doujinshi came out? Which is quite a lot of them, really. It's got its share of 18+ material, but that's hardly the majority of content that fans have made.

Touhou doujins are best suited to comedy, I think. Should that also touch on some more mature themes, then all the better.
Like AoI, for example.

Indeed. XD Well, with so many, there almost has to be... which certainly isn't a bad thing. Sometimes the weirdest combinations are the best.
