There's a special attachment that goes into the GBA cartridge with the four buttons (Yup, only four). You strum with the stylus on the second screen.

grrrrr. i just rented a movie from itunes, and it turns out it won't work with my 5th gen ipod. WHY couldn't they tell me this BEFORE i rented the stupid thing???
