Hm,well full out sex scenes(meaning,all the noises and so forth that come about when people have sex) or nudity(meaning,no censorship of the genitals on any gender) for one thing...
Over use of profanity and violence can also make something mature,but usually that can keep it in the Teens.
When something is really gory it gets rated mature.
But it's usually shoved into mature if something what I said above in the first sentence,from what I noticed.
