It's where you create your own costume to (traditionally) fit your favorite anime character.
Many people who go to conventions do this.

I've never went to a convention, but I did create a Shuyin costume for Halloween a few years ago. It's just something fun a lot of Otaku's do.
Some people go very far overboard - spending hundreds of dollars for their outfits.
