Vampire, that way I can live for a long time and out live my enemies.

Ouch, um, eaten by werewolf, I guess. Probably be over quicker.

Head. Again, it would be quicker.

Depends on which version of the legends you're talking about. But based on modern tales, vampire.

Oooh, tough one. Skeleton, I guess.

Stabbed repeatedly or hit with a metal bat repeatedly?
Depends on where I was getting hit, but I guess stabbed. If I died from it, it'd be quicker, and if not, at least you know where all the injuries are.
