A. i got it from my sister, in her profile's about there is 2 links
to a tutorial, those will help yoo.

A. I made it with photoimpact and designed it in my own way

A. every other month or week i'll make a different header for my profile.

A. Again with photoimpact, it lets me make different
designs on a picture.
